There’s tons of games that are surreal, and trippy. It all depends on your perspective, on what makes something trippy and surreal.
Also Xenogears (one of my favs) i think is quasi-surreal. Such as the scene with an all black screen, and ominous music playing, and the text comes up and says “I am the alpha and the omega…”
I think that whole game is trippy/surreal…just the way everything falls together…and the whole plot about solaris, and everything.

Is Koudelka worth getting? I’ve heard a lot about it but never seen any reviews…
What about Frequency. That was pretty surreal if you got into it fully. Your fingers dont know what they’re doing. 0_o
In my opinion Koudelka is worth getting at the price you can buy it for. Hell, I was making a shrine for it until I lost my first disc ;_; (Which I’m still trying to find). It is slightly hard to find here in England now though.
But they were also pure crap, and are not worth mentioning in any coversation ever. Just my opinion. Graphic ‘Adventures’ are usually lame, unless they’re made by Lucasarts.

ICO and Archangel.
Both of those games seem more like dreams to me. ICO has the blind princess meeting a horned boy and trying to escape from shadow beings. It’s not so much the story as it is the general atmosphere and the look of things.
Same thing with Archangel. It’s all angels and demons, but the angels don’t look like the typical representation. They’re more flowing, ethereal beings with wisps of energy for wings. It has a decent polygon count, but the details aren’t high, and it helps with the atmosphere. A lot of the world is obscured by fog, also.
I played a text-based game of Hitchhiker’s Guide to the Galaxy that was pretty trippy. Also, I’ve always felt Lemmings was definitely surreal in a “hooray, this is so peppy and fun…hundreds of lemmings fall to their deaths! ^^” And remember Mischief Makers? THAT game was definitely drug-induced. Best. Game. Ever. But still drug-induced.

Oh yeah, and Maniac Mansion for the NES. Definitely made by designers on drugs. I have no question about it, and it’s not even a joke. Actually most games for the NES were made by people intoxicated in one way or another (including Kirby’s Adventure, which actually is the best, most solid, most untiringly fun game ever made).
