"FDA Issues Warning on Antidepressants

Possible Suicide, Severe Depression, Anxiety, Panic Attacks in Children and Adults

March 22, 2004 – The FDA is issuing a warning about the possibility of worsening depression or suicidal thoughts in people, particularly children, who take any of 10 popular antidepressants, especially at the beginning of treatment or when the doses are increased or decreased.

The FDA has sent a letter to drug manufacturers requesting labeling changes on these antidepressants – warning of possible suicide, worsening depression, anxiety, and panic attacks in adults and children.

Antidepressants involved in this warning label request are:

Prozac (also sold generically as fluoxetine)
Zoloft
Paxil
Luvox
Celexa
Lexapro
Wellbutrin
Effexor
Serzone
Remeron

“We don’t know that the drugs are responsible for these behavioral changes, but nonetheless we’re telling physicians and families to be aware of this and that if the behaviors do emerge, to get treatment right away,” said Russell Katz, a director with the FDA’s Center for Drug Evaluation and Research, in a news teleconference today.

The proposed warning label will “include information about behavioral changes that may occur in patients who are prescribed antidepressant drugs,” said Katz.

“This applies to adult and pediatric patients and involves the potential for suicidal thinking or suicidal behaviors and warns the physician and family about any behaviors that might emerge that haven’t been experienced before,” he added.

But the FDA stopped short of recommending people discontinue taking their antidepressants.

“We specifically decided not to tell people not to use these drugs,” said Katz. “We don’t think that is necessary at this point.”

In addition to looking for signs of worsening depression, the following symptoms may also be a sign of a problem:

Anxiety
Agitation
Panic attacks
Insomnia
Irritability
Hostility
Impulsivity
Severe restlessness
Mania in both adults and children being treated with antidepressants for major depression

If these changes appear, treatment should be evaluated, the FDA says. Medications may need to be discontinued when symptoms are severe, begin abruptly, or if they signal a new disorder.

There also is concern for people who have bipolar disorder (manic depression) but don’t know it. Antidepressants have the potential for provoking a manic episode in these people, the FDA says. Doctors, patients, and family members should be on the lookout for any symptoms of mania, including feeling extremely happy or very irritable, inflated self-esteem, not needing as much sleep as usual, talking, or being more active than usual.

Reviewing Studies of Children

The FDA has been closely reviewing studies of antidepressants in children for the past year after an initial report on studies of Paxil and other drugs suggested an increased risk of suicidal thoughts in children given antidepressants. No suicides occurred in any of the trials, the FDA reports.

The analysis has involved 25 controlled trials with 4,000 children taking antidepressants, said Katz. “Those reviews are ongoing.”

This action “arose from an unexpected observation in some studies of an apparent excess of emotionability, a catch-all term … that includes suicidal [thoughts],” said Robert Temple, MD, the FDA’s director of medical policy.

However, closer examination has shown that the accounts are unclear – whether certain behaviors reported were actual suicide attempts or other self-injurious behavior that was not suicide-related.

“As we’ve gone into the studies in detail, it’s become evident that the terms used were highly varied and [the studies] not very well done,” Temple said.

Experts at Columbia University in New York are providing an independent review of the studies, looking carefully at descriptions of these events to determine whether those cases represent suicidal behavior.

“We hope to have a much better idea whether these drugs are effective and whether or not they do cause suicidal symptoms,” said Katz.

“There is an ongoing awareness that children are particularly vulnerable, that they need additional protection, and that we make sure that all the protections are in place,” said Dianne Murphy, MD, the FDA’s director of Pediatric Therapeutics."

Lol, what's wrong with this picture? I've taken prozac, wellbutrin, and i'm currently on celexa. I've never had anxiety, agitation, panic attacks, irritability, and impulsivity(sarcasm). Wellbutrin is powerful though as i took 2 pills(cant remember my dosage then) in the morning and by afternoon i had a seizure. I was supposed to space the time out between the pills opps. I'm good now though, i'm out of high school. ^ _ ^

I actually knew about most of this stuff. The Church my mom works for does a lot of stuff against the phyciatrists and the drugging of people (esp. children).

I agree for the most part with what she believes, but I also think they may, at times work (but keep in mind so does a sugar pill).

The drugs aren’t worth the possible side effects, and some of them you can’t just “quit taking”, you have to switch to a less powerful drug first.

Also, the thing that hits home: there’s no scientific proof of drugs actually helping anything. Also, there is no scientific proof of ADAH and ADD even existing.

Some suspect it isn’t the anti-depressents but the underlying cause as to why they are taking the anti-depressents in the first place.

Sort of like, after everyone who has a cough takes cough drops eventually someone blames the cough on the cough drops…

Interesting thought Chris_Starshade, i reccommend therapy more then pills. I’ve always been a little iffy on taking pills, since is it making me a different person then i really am? Who knows what else the pills could be doing to me, but i don’t think i’ve had any noticeable problems ever with them. With therapy you have to make sure the therapist is working with you on problems you want to work on in the order you want. I found out the hard way as i was with a therapist for 4 years and didn’t ask myself why i was still in the same rut. She was trying to help me with school, when i wanted to help myself out of being semi depressed and etcera first.

I found it interesting that sugar pills work, as its something if you think your’e gonna get better you will, guess maybe motivational speeches and such could really work as well. Often i am saying after drug commericals air on tv, oh what lovely side effects. Some side effects one could say forgot those pills i’m not gonna be puking my guts out and having constant nosebleeds, etcera.

Well with ADD and ADHD, seems all kids have the problem. My thinking is the cause is parents let their kids learn from watching tv. Thus kids need televison to teach them anything, and some boring teacher most likely is not going to have happy puppets and colors. Seems like a cop out to blame tv, i know, but its not my only theory, the other is plain bad genetics or kids are just kids always jumpy. Otherwise as usual i dont have a clue. Plus kids are always wanting to talk to their friends and they somehow can’t wait till lunch or after school, i never got that, but i never had that many friends. However its hard to find good amusing teachers as well. I’ve never been on any pills that i’ve had trouble getting off of, though i’ve never really stopped, sometimes skipped a day, always have had low dosages. Don’t know whether some drugs work or not, but wonder if i could have lived without them.

Heh. My doctors put me on Prozac and Ritalin at the same time when I was in third or fourth grade. That was quite possibly the stupidest fucking decision they ever made. =p

I just saw something on the noon news, about prozac and other such drugs having a high suicide rate. That was just 5 minutes ago or so…is the truth about these drugs coming out all of a sudden?

The drugs are given to treat depression. Like starshade said, it might be that blaming the suicide on the drugs is like blaming colds on cough drops.

Also note that all the news agencies pretty much repeat the same story over and over again. Just because it’s been reported on the news 10 times doesn’t mean 10 different studies have come to the same conclusion.

Actually, almost all of the school shootings in the past few years have been linked to drugs like prozac. (Columbine, for example…or however you spell it.)

I do think that from time-to-time the drugs do help, but I think they’re a lot more harmful than helpful.

Also, I forget where, but Paxil (another of these drugs) has recently been outlawed because it was shown that people who have taken it have been 3 times more likely to kill themselves. And no, they didn’t outlaw the drug based only on theory; they had scientific proof.

Maybe its just a sign of the times, but generally anytime is a good time to try suicide for those who want to. Or more and more people don’t care about others, or doing stupid stunts or killing themselves over something silly perhaps.

Effects vary greatly from person to person, and most of the side effects don’t happen, at least in my experience. For instance, the psychiatrist prescribed Paxil (as opposed to other drugs) for me because I have frequent insomnia and one of the side effects is supposedly drowsiness, so I take it before bed time. It has yet to cause me any drowsiness.

And while I was initially doubtful that it would help any, I have been feeling much better since I began taking the medication.

Edit: Interestingly enough, my Economics teacher discussed a very similar article today in class.

I used to be on Zoloft and Ritalin.

Can’t remember if it was working or not though. Don’t think it was, since I still remember Middle School as Hell on Earth.
